Yes, people have been noticing this on all sites throughout the Stack Exchange network. There's a 'central' bug post on the Main Meta which has now been marked status-completed, meaning it should be resolved.

Given these search results, I would say that the phenomenon is far from rare. There are 288 posts with at least 10,000 views with a score of 3 or lower. My cutoffs were arbitrary there, but you can play with the parameters and see that there are many posts with lots of page views and low scores.

I don't think that there is a progress marking for the easier to achieve badges. You can see things on the review tab where the longer to achieve tasks do get progress bars. Badges can take a while to award. The 10 hours wait lets you know when the daiy script will run and post a badge that gets checked but once a day. Let us know if it's not happened in a dozen minutes after you can vote again.

Badges on Meta mean different things — for example, the answer badges on Meta means that you've answered a question regarding how the site functions, not the site itself. This is substantially different due to the difference in subject matter. As an aside, reputation is tracked independently on Meta, just not shown.

I think some questions/answers I've down or upvoted have been removed. Because if I count the questions and answers right now, it's missing 4. I think that's why I didn't get the badge.

In my eyes they are related, and just a degree of how sustained your moderation efforts on the site relate to the voting buttons.
